How to administer suppositories

Administering a suppository is a common clinical skill tested in the NMC OSCE (Objective Structured Clinical Examination) for nurses in the UK. This procedure is essential for delivering medication directly into the rectum, where it can be absorbed quickly into the bloodstream or act locally. Here’s a step-by-step guide to help you master this skill.

Preparation

  1. Confirm Patient Identity and Consent
    • Verify the patient’s identity using their wristband and ask them to confirm their name and date of birth.
    • Explain the procedure, its purpose, and any potential side effects. Obtain informed consent.
  2. Check Medication and Equipment
    • Review the prescription chart to ensure the correct medication, dose, and route.
    • Gather necessary equipment: gloves, lubricant, suppository, and a disposable pad.
  3. Hand Hygiene and PPE
    • Perform hand hygiene thoroughly.
    • Wear appropriate personal protective equipment (PPE), including gloves.

Procedure

  1. Position the Patient
    • Assist the patient into the left lateral position with their knees slightly bent. This position helps to facilitate the insertion of the suppository.
  2. Inspect the Area
    • Check the perianal area for any abnormalities, such as hemorrhoids or infections, which might affect the procedure.
  3. Lubricate the Suppository
    • Apply a small amount of water-based lubricant to the suppository to ease insertion.
  4. Insert the Suppository
    • Gently separate the buttocks and insert the suppository, blunt end first, about 2 to 4 cm into the rectum. Ensure it is placed against the rectal wall to prevent it from slipping out.
  5. Post-Insertion Care
    • Advise the patient to remain lying down for at least 20 minutes to allow the suppository to dissolve and be absorbed.
    • Provide the patient with a call bell and ensure they are comfortable.

Aftercare

  1. Dispose of Waste
    • Dispose of gloves and any waste materials in the appropriate clinical waste bin.
    • Perform hand hygiene again.
  2. Documentation
    • Document the administration of the suppository in the patient’s medical records, including the time, medication, dose, and any observations.
  3. Monitor the Patient
    • Monitor the patient for any adverse reactions or side effects and provide assistance as needed.

Tips for Success

  • Communication: Clearly explain each step to the patient to reduce anxiety and ensure cooperation.
  • Gentleness: Be gentle and patient during the insertion to minimize discomfort.
  • Observation: Always observe the patient for any signs of distress or discomfort during and after the procedure.
